Authorization for Service Termination

I, the undersigned Distributel customer, hereby authorize I.D.Logic Inc. to act on my behalf with respect to the termination of my telecommunications services with Distributel.

I understand that this authorization will allow I.D.Logic Inc Telecommunications Provider to initiate the process of terminating my services, including the cutting of the telephone line and other related services, according to the conditions agreed upon between me and Distributel Telecommunications Provider. I also take responsibility for any fee or cost associated with this termination.

I request that you provide all necessary assistance to I.D.Logic Inc Telecommunications Provider to facilitate this termination process, and confirm that I am fully informed of the consequences of this decision.
